An example of a Questionnaire

Questionnaires are ways of finding out information about individuals on the database.  It is however very important that you target who you want to send questionnaires to and that you have a process of dealing with the information in a prompt and efficient manner.  The information provided by individuals are quite detailed and should be acted on promptly.

You are also subject to the data protection act and you need to check how long you are able to hold on to this information and use it.
 

This are some suggestions on a questionnaire.  Some questions will not be applicable and are put in there to jog your memory:-
